COURSE OVERVIEW:

This comprehensive course on Root Cause Analysis (RCA) provides participants with a structured, practical framework for understanding, investigating and resolving failures in industrial and organizational environments. The program explains how to examine and prioritize failures, store and analyze failure data, and order a systematic failure analysis that leads to sustainable solutions rather than temporary fixes.

 

Participants will learn how to develop and implement effective RCA programs that are integrated into maintenance, operations, safety and quality management systems. The course emphasizes making RCA a structured, team based and analytical approach, rather than an informal, ad hoc activity. It also highlights how to identify organizational deficiencies in failure analysis processes, such as poor communication, weak reporting systems and lack of accountability.

 

Through real world examples, tools and case studies, the course demonstrates how to apply RCA correctly to alleviate repeated failure problems in a facility, reduce downtime, improve reliability, strengthen safety performance and support continuous improvement.

 

COURSE OBJECTIVES:

After completion of this course, the participants will be able to:

  1. Explain the principles and terminology of Root Cause Analysis and General System Failure Analysis.
  2. Examine and prioritize failures based on risk, cost, safety and operational impact.
  3. Collect, store and structure failure data in a way that supports effective analysis and learning.
  4. Develop and implement effective RCA programs that are aligned with organizational goals and management systems.
  5. Apply structured, team based and analytical RCA methods to investigate failures and near misses.
  6. Identify the roles that people and processes play in failure mechanisms and problem solving.
  7. Recognize organizational deficiencies that hinder effective failure analysis and propose improvements.
  8. Follow a step-by-step RCA process, from problem definition through evidence collection, analysis, identification of root causes and development of corrective actions.
  9. Develop practical, cost-effective corrective and preventive actions that address root causes rather than symptoms.
  10. Evaluate the effectiveness of RCA investigations and embed lessons learned into procedures, training and standards.

TRAINING COURSE METHODOLOGY:

A highly interactive combination of lectures and discussion sessions, as well as case studies, will be managed to maximize the amount and quality of information, knowledge, and experience transfer. The course will be intensive but practical and highly interactive. The sessions will start by raising the most relevant questions and motivating everybody to find the right answers. The attendants will also be encouraged to raise more of their questions and to share in developing the right answers using their analysis and experience. There will also be some indoor experiential activities to enhance learning. Course material will be provided in PowerPoint, with necessary animations, learning videos, and general discussions.

 

The course participants shall be evaluated before, during, and after the course.

 

COURSE CERTIFICATE:

National Consultant Centre for Training LLC (NCC) will issue an Attendance Certificate to all participants completing a minimum of 80% of the total attendance time requirement.
